Support for families with children aged 6 to 16 with intellectual and developmental disabilities.

Families with children who are living and enrolled in school in Northumberland County
qualify for Children’s Services. Services include:

Transition

Support to successfully manage life transitions and build family capacity through the development of a plan for the family and child

Navigate

Assisting families to navigate available services, resources, and supports

Plan

Ongoing planning, securing and leveraging of available resources, support, and funding

Link

Liaising with the school system and other children’s services and organizations

Building Bridges

Building Bridges services are available to students with intellectual and developmental disabilities aged 16 to 21.

Students must be living and enrolled in school in West Northumberland. Services include:

Transition

Supporting students to transition from school to adult life

Planning

Assisting students to direct and develop an individualized transition plan in conjunction with people most important to the students

Explore

Assisting students to explore and identify future community options available after high school, including post-secondary education, employment, volunteering, and social and recreational opportunities

Secure

Securing and leveraging available resources, supports, and funding

Link

Liaising with the school system and other youth services and organizations

Children’s Respite

Children’s Respite is a resource for families with children aged 0-17 seeking respite services.

Service coordination and case management support is provided to help families with:

planning respite needs

completing an Individual Support Plan that is person-directed to build on goals

accessing and understanding funding options

exploring the full range of community-based services

completing service and/or funding applications

attending school meetings and/or case conferences as required

Person Directed Planning

Person Directed Planning is offered on a fee for service basis for people who are not connected to an agency that provides this service.

Over a thirty-eight-hour period, a facilitator will engage with the person requiring support and relevant family members or loved ones to develop a profile from engagement and observation, facilitate a series of planning meetings, develop a person-directed plan, and an implementation process with appropriate connections and resources to implement it. The service also includes securing and leveraging available resources, supports, and funding.

Individualized Funding Brokerage

Support for people and families in managing individualized funding such as Passport funding.

Brokerage support ensures all reporting, procedures, and accountability required by the Ministry of Children, Community and Social Services are met. A 10% administration fee will apply.

Funding

Managing individualized funding such as Special Services at Home, Flex Funds, or Passport Funding

Development

Developing an Individual Support Plan (if required), which identifies the child’s or youth’s goals and wishes

Connection

Connecting to other community agencies and service providers who can provide the supports necessary to utilize available resources and accomplish identified goals
